Is the person you are enquiring about:

  • Over 75 years old?
  • Have they been recently discharged from hospital?
  • Have they experienced a recent change in their cognitive status?
  • Do they live alone?
  • Have their medical conditions worsened recently?
  • Are they experiencing increasing frailty?
  • Is their main carer or next-of-kin under stress?
  • Experienced a fall recently?
  • Are they vulnerable to safety risks?

Your Level of Care

  • Basic Care

    For people who need a little extra help to continue to live independently at home, typically requiring one or two visits per week.

  • Low Level Care

    For people who need care to continue to live independently at home, typically requiring three or four visits per week.

  • Intermediate Care

    For people who need care to help them remain at home longer, typically requiring visits most days of the week and possibly nursing.

  • High Level Care

    For people who need care to help them remain at home longer, typically requiring medical or nursing support every day of the week.
